This let’s us record the client and patient information into our management software so that we can effectively and efficiently provide the service each client deserves.  It fulfills our legal requirement of recording keeping and is a mandatory form to be completed by every client.  Click on the form below to download and print a copy of each form.  Filling out the required forms prior to bringing your pet in will save you time!

Patient Drop-Off Questionnaire

We value our clients and patients!  We understand that even though your pet is sick you can't afford to take a day off work or you may have to drop off the kids at school or soccer practice.  We appreciate that your life can be very busy and to accomodate that you can drop off your pet at our hospital for evaluation while you attend to tasks of your day.  Upon examination of your pet a member of our staff will contact you with an estimate for recommended diagnostics and treatment.  As a valued client the only charge to you is our regular consultation fee for the examination of your pet, there is no additional fee for hospitalization or boarding.  Unfortunately we cannot offer this service for euthanasia cases, emergency or critically ill patients.  We hope that this service will make your day a little easier.  Filling out the Patient Drop-Off Questionnaire form prior to bringing your pet in will save you time!

Boarding Release and Regulations - Required

This let’s us know the current state of your pet's health and any medical and nutritional necessities that may be required to provide proper care for your pet while boarding.  It fulfills our legal requirement of recording keeping and is a mandatory form to be completed by every client that is boarding a pet at Arlington Animal Hospital.  Click on the form below to download and print a copy of each form.  Filling out the required form prior to bringing your pet in will save you time!

Member Wellness Registration Form - Optional

This form can be filled out to register for access to the member wellness section of our website. You can also sign up for our monthly newsletter to keep up on current health issues and news and events in our office.
